Do I need my iPhone?

Not while you are using it. This is a standalone watchOS app: it runs on the Apple Watch on its own, with the phone switched off or left behind. You do need an iPhone once, to purchase and install it — that is an App Store rule rather than anything about this app.

Is the Apple Watch microphone accurate enough to tune with?

For standard tuning at normal playing volume, yes. In a loud room, mute other sources and play a single string firmly - the same thing you would do with any microphone tuner.

Does it need my iPhone nearby?

No, it works standalone. That is the entire point of it.

Can I tune to something other than A440?

It detects notes chromatically, so alternate and drop tunings work without configuration.

Does it work with a clip-on pickup?

No, it listens through the watch microphone. Acoustic sound is what it needs.
